Building an Infernal Machine

In order to construct the Infernal Machine, and ultimately acquire a coveted Hellfire Ring (Strength, Dexterity, Intelligence, Vitality) you need to gather some keys, organs and designs that will take you through each of the 4 acts. To collect the components, you need to be in Inferno difficulty and have a 5 stack of Nephalem Valor. Monster Power comes into play here as well, as each point of monster power increases your odds by 10% (at MP 0 you only have a 5% chance). If you can handle it, MP 10 will guarantee you the drop every time.

Gathering Keys

Each of the keys drop from unique keywarden carged with powerful monster affixes.

Key of Destruction - Act I

The first key drops from Odeg the Keywarden who spawns in the Fields of Misery in Act I. Start the game with the "Return to New Tristram" quest selected. I recommend heading to The Festering Woods and to hit up Warrior's Rest and Crypt of the Ancients for a couple quick stacks. You can usually gather 3-4 stacks within a few minutes here. After you're done in The Festering Woods you can go ahead and travel to the Fields of Misery. While looking for the Keywarden you should find the remaining packs to get your 5 stacks. Even if you find him, don't kill him before you're at 5 stacks or you won't get the key drop.

This guy isn't too difficult, the only 2 things you need to watch out for is that he is Molten and does a periodic Fireball Volley.

Key of Hate - Act II

Your next key drops from Sokahr the Keywarden in Dahlgur Oasis in Act II. Start the game with "The Black Soulstone" quest selected. Quick stacks of Nephalem Valor can be obtained from Vault of the Assassin in Desolate Sands. It can be difficult to find it, but once you locate it you'll get a checkpoint so when you leave the game you're back at the Vault (good for repetitive key farming in the act). Once you have 3 or 4 stacks head to Dahlgar Oasis and search for the Keywarden. He can be tricky to find so there's a good chance you'll have 5 stacks before you run into him.

Sokahr has the Missle Dampening affix and will pull you closer using Vortex while constantly spamming Mortars.

Key of Terror - Act III

The final key drops from Xah'Rith the Keywarden at Stonefort in Act III. Start the game on the "Heart of Sin" quest. If you're familiar with the infamous Alkaizer, you can rack up 5 stacks of Nephalem Valor fairly quickly. Clear the Core of Arreat up to the entrance to Azmodan for the checkpoint, then clear Tower of the Damned Level 1 and Arreat Crater Level 2. By this point you should have 5 stacks of Nephalem Valor in a short period of time.

Xah'Rith can be the most troublesome of the 3. His main mechanics are his Teleport and Illusionist abilities. He will also drop frozen corpses on top of you, so when you see the animation below your feet you have a second or two to move before your character is slowed.

Getting the Plan

Plan: Infernal Machine allows you to combine your keys into the machine at your Blacksmith. Luckily you only need to farm the plan once to learn the recipe.

The plan drops from Nekarat the Keywarden in the Silver Spire Level 1 in Act IV. You should start the game on the quest "Prime Evil". The worst part of Act IV is how short it is and the lack of elites overall to gain a 5 stacks of Nephalem Valor, so you pretty much have to begin at the start of the act and work your way forward. Once you gain 5 stacks, take The Crystal Colonnade waypoint and make your way to the Silver Spire Level 1 to find the Keywarden.

The Infernal Machine

Once you have a set of 3 keys and have learned the plan on your Blacksmith, you're ready to craft the Infernal Machine at a bank-breaking cost of 12,000 gold. In order to craft it your Blacksmith needs to be level 10.

Now that you have an Infernal Machine, make sure you're on the quest "Return to New Tristram" (the one after you defeat The Butcher). Locate Brother Malachi the Healer in north eastern part of town. The door on the house near him will break open to allow you to enter Heretic's Abode.

Important: Farm up 5 stacks of Nephalem Valor before you use your Internal Machine!

Once inside Heretic's Abode you can click on The Infernal Machine item in your inventory and a random portal to one of the three realms will spawn. Each realm can only spawn once per game, so your best bet is having 3 machines to guarantee a portal to each will spawn once. Oh, remember to bring friends! Farming with others will reduce the time investment for everybody, and your loot is your own so there's no consequence to it.

Organs

Each realm houses 2 bosses you've conquered previously, however, they will now be much more difficult, "uber" even! Realm of Discord will have you face King Leoric and Maghda, Realm of Chaos has Ghom and Rakanoth, and Realm of Turmoil has Siege Breaker and Zulton Kulle. After you defeat each duo you have a chance at getting an organ, and again the likelyhood of the drops increase by 10% for each point of Monster Power you play with.

Hellfire Ring

Now that you are the proud owner of 3 pieces of monster innards, it's time to combine them into something worthy of all this effort. Head to Act II and speak with Squirt the Pedller to purchase Design: Hellfire Ring for 2 million gold. Talk to your Jeweler to learn this design.

You can now craft the Hellfire Ring of your choice (Strength, Intelligence, Dexterity or Vitality). The craft costs 50,000 gold and each ring will give you 35% bonus experience, 170-200 of the primary stat you selected, a chance to cast a fireball whenever you attack and 4 random properties (may the odds ever be in your favor). If you aren't happy with the random stats, you can go through this whole process again as many times as you want. Hellfire Rings are account-bound and therefore cannot be sold and are unique-equiped, though your follower can equip one as well.
